Mandatory form for all clients wishing to apply for the basic Canada Education Savings Grant (CESG), the additional CESG or the Canada Learning Bond (CLB).

Must be completed when the subscriber is not the primary caregiver and/or the custodial parent/legal guardian of the child.

There are 2 possible scenarios:

1. The primary caregiver is also the child’s custodial parent or legal guardian: the primary caregiver must complete Annex B.
2. The primary caregiver is not the child’s custodial parent or legal guardian: the primary caregiver AND the custodial parent/legal guardian must each complete a separate copy of Annex B.

The form Privacy and Protection of Personal Information must be completed by the primary caregiver and/or custodial parent/legal guardian who completes Annex B of the Application for the Basic CESG, Additional CESG and CLB. In all cases where Annex B is completed, the form Privacy and Protection of Personal Information must also be completed and attached thereto.

We remind you that receiving the SIN is mandatory for the registration of your education savings plan and for your child to receive the government grants.
